El derrame pleural es una condición frecuente, caracterizada por la acumulación patológica de líquido en el espacio pleural. En México, los estudios sobre la epidemiología del derrame pleural provienen principalmente de hospitales de tercer nivel, los cuales tienden a recibir casos más complejos y cuentan con acceso a estudios diagnósticos avanzados. Objetivo: Describir las características clínicas, radiológicas y del líquido pleural de pacientes con derrame pleural en un hospital de segundo nivel de atención del noreste de México. Materiales y métodos: Estudio transversal, retrospectivo, descriptivo. Se incluyeron pacientes hospitalizados en el Hospital General de Zona con Medicina Familiar No. 2 de la ciudad de Saltillo, Coahuila entre noviembre 2024 y junio 2025 con derrame pleural confirmado por toracocentesis. Resultados: Se analizaron 48 casos, el 56.3% fueron hombres, la media de edad fue de 61.1±14.49 años. El derrame se presentó de forma unilateral en 93.7% con predominio de lado derecho (71.1%). El 68.7% de los derrames fueron exudados; las principales etiologías fueron paraneumónicos (27.1%), y asociado a neoplasias (22.9%). El 18.8% de los pacientes tuvieron un desenlace fatal y el 8.3% presentaron recurrencia del derrame pleural. Conclusiones: Los derrames pleurales en el hospital general de segundo nivel mostraron un predominio de causas paraneumónicas y neoplásicas, con alta frecuencia de comorbilidades asociadas y una mortalidad hospitalaria relevante.
